when they do submissive smile it could also mean they will attack defensivly

I believe you're thinking of the long snarl, not the submissive smile. The submissive smile is used as an appeasement gesture and when the wolf's feeling apprehensive or fearful. It can then escalate to the long snarl, which is indicative of a wolf ready to defend itself if pressed much further.
